One of the ways we’ll be able to help honor your time is to have completed the New Patient Forms before or immediately upon your arrival. You can download and print the New Patient Information Form here. Please remember to bring it with you at the time of your appointment or mail the completed form back to us as soon as possible. If you are unable to complete it before arrival, please arrive to your appointment 15 minutes early.

We feel it is necessary to develop a rapport with our patients. Many new patients have had a past unpleasant dental experience. It is crucial to us to know and understand your concerns, fears and dental expectations. The Handle With Care Form that will take just seconds to complete, but allow us to serve you with the highest quality and sensitivity. You’ll be surprised, once you visit Dr. Rice, how many of these concerns are already handled with utmost care and considered a standard for service. However, we want you to have a chance to express yourself before your visit anyway and this form allows you that opportunity. We know it’s important to feel like you’re being heard, and our number one goal is to listen and serve accordingly!
